Description

Job Title: Physical Therapist (PT) – Home Health
Location: Morgantown, WV 26508
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No weekends, no on-call)
Evaluate and treat patients per physician orders
Develop and adjust individualized treatment plans
Monitor patient progress and modify interventions as needed
Communicate with physicians, care teams, patients, and families
Recommend adaptive equipment to improve patient function
Maintain documentation and supervise PTAs/Aides
Participate in staff development and continuing education

Requirements

WV Physical Therapist License
Home health experience, including OASIS documentation
Homecare Homebase (HCHB) experience preferred
