Job Title: Physical Therapist (PT) – Home Health
Location: Ronceverte, WV 24970
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No weekends, no on-call)
Mileage Reimbursement: $0.49 per mile
Evaluate and treat patients per physician orders
Develop and adjust individualized treatment plans
Assess patient progress and modify interventions as needed
Communicate with physicians, care teams, patients, and families
Recommend adaptive equipment to enhance function
Maintain accurate documentation and supervise PTAs/Aides
Participate in staff development and continuing education - Requirements

WV Physical Therapist License (Compact license accepted)
Home health/homecare experience, including OASIS documentation
Knowledge of exercise programming and rehabilitation techniques
Homecare Homebase (HCHB) experience preferred - Location
