Physical Therapist – Home Health (Contract)
Location: Clarksburg, WV
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Hours: 40 hrs/week


Job Overview:
We're seeking a licensed Physical Therapist to provide home health services in the Clarksburg, WV area. This role focuses on evaluating patients, delivering individualized rehabilitation care, and improving patient function and independence in the home setting.

Coverage Area:
Harrison, Marion, Taylor, Barbour, Upshur, Lewis, and Doddridge counties

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop and update individualized treatment plans
Provide therapeutic exercises and functional rehab in patient homes
Communicate patient progress with physicians and care teams
Complete accurate and timely documentation
Supervise PTAs and aides as required

Active West Virginia PT license
Home health experience preferred
OASIS and HCHB experience a plus
