Job Title: Physical Therapist (PT) – Home Health
Location: Clarksburg, WV 26323
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No weekends, no on-call)
Perform physical therapy evaluations and treatments based on physician orders/referrals
Conduct comprehensive patient assessments using appropriate evaluation tools and procedures
Review patient records for diagnosis, medical history, precautions, and contraindications
Develop measurable treatment plans to maximize patient independence and function
Reassess patient progress regularly and adjust treatment plans as needed
Communicate patient needs and progress to physicians, interdisciplinary teams, patients, and family members
Recommend and assist in obtaining adaptive equipment to enhance patient function - Requirements

WV Physical Therapist License
Home health experience, including OASIS documentation
Homecare Homebase (HCHB) experience preferred - Location
