Location: Southwest Oklahoma City Communities (Tuttle, Blanchard, Purcell, Newcastle, West Moore)
Salary: $80k-90k/yr. (based on experience)
Productivity Bonus: $40 for each point earned over 36 points per week

About the Opportunity
A well-established Home Health & Hospice organization is seeking an experienced and compassionate Field Hospice Registered Nurse (RN) to serve patients throughout the Southwest Oklahoma City communities (Tuttle, Blanchard, Purcell, Newcastle, West Moore).
Key Responsibilities
-Provide skilled hospice nursing care to patients in their homes in accordance with physician orders and individualized plans of care
-Focus on patient comfort, dignity, symptom management, and quality of life
-Perform comprehensive patient assessments and develop and update individualized plans of care
-Monitor patient conditions and communicate changes to the appropriate members of the care team
-Educate patients and families regarding medications, treatment plans, symptom management, and available resources
-Collaborate with physicians and interdisciplinary team members, including social workers, therapists, and other healthcare professionals
-Provide emotional support and guidance to patients and their families throughout the hospice care process
-Accurately complete clinical documentation in accordance with agency, state, and federal guidelines
-Coordinate care and communicate effectively with patients, families, caregivers, and members of the interdisciplinary team
Perform other duties as assigned

-Reliable transportation and a valid driver's license are required for home visits
