Salary: $80k-87k/yr. (based on experience)
Productivity Bonus: $40 for each point earned over 36 points per week

About the Opportunity
A well-established Home Health & Hospice organization is seeking an experienced and compassionate Field Hospice Registered Nurse (RN) to serve patients throughout the Southwest Oklahoma City communities (Tuttle, Blanchard, Purcell, Newcastle, West Moore).
Key Responsibilities
Provide skilled hospice nursing care to patients in their homes in accordance with physician orders and individualized plans of care
Focus on patient comfort, dignity, symptom management, and quality of life
Perform comprehensive patient assessments and develop and update individualized plans of care
Monitor patient conditions and communicate changes to the appropriate members of the care team
Educate patients and families regarding medications, treatment plans, symptom management, and available resources
Collaborate with physicians and interdisciplinary team members, including social workers, therapists, and other healthcare professionals
Provide emotional support and guidance to patients and their families throughout the hospice care process
Accurately complete clinical documentation in accordance with agency, state, and federal guidelines
Coordinate care and communicate effectively with patients, families, caregivers, and members of the interdisciplinary team
Perform other duties as assigned

Minimum of 3 years of nursing experience
3 years of Hospice nursing experience is required
Hospice experience required
Strong clinical assessment and patient care skills
Ability to work independently and effectively manage a home-based patient caseload
Reliable transportation and a valid driver's license are required for home visits
