Loving Care In-Home Health Services is a family-owned home health, hospice and private duty services provider since 1997. Our health care services are available to people in 16 counties within the central Oklahoma area.

We are currently accepting applications for a full-time RN Home Health Case Manager for our Yukon Home Health office. This position will be responsible for delivery and coordination of care to assigned home health patients. This area will include Yukon, Mustang, Tuttle, Piedmont, NW OKC, Bethany, El Reno, Edmond and surrounding areas.

ABILITIES:
Must be able to manage and coordinate all aspects of patient care to assigned home health patients;
Must be able to work with established home health care team;
Must have ability to communicate and interact with patients and family members;
Must possess good computer skills;
Must be able to work independently and organize/prioritize schedule.

EXPERIENCE:
Home health: 1 year (Preferred)
Case Management: 1 year (Preferred)
Nursing: 1 year (Required)

Active State of Oklahoma RN license without restrictions;
Current CPR certification;
Current valid Drivers License;
Current required Automobile Insurance requirements
