We are currently accepting applications for a full-time RN Home Health Case Manager for our Shawnee Home Health branch office. This position will be responsible for delivery and coordination of care to assigned home health patients. The areas to be serviced will include Shawnee, Tecumseh, Prague, Choctaw, Harrah, Seminole, Wewoka and surrounding areas.

ABILITIES:
-Must be able to manage and coordinate all aspects of patient
care to assigned home health patients;
-Must be able to work with established home health care team;
-Must have ability to communicate and interact with patients
and family members;
-Must possess good computer skills;
-Must be able to work independently and organize/prioritize
schedule.

Experience:
Home health: 1 year (Preferred)
Case Management: 1 year (Preferred)
Nursing: 1 year (Required)
