* The qualified candidate will have 2 years of experience performing OASIS assessments and care planning for in-home patients.

DUTIES & RESPONSIBILITIES:

* Responsible for the care of patients in their home.

* Perform OASIS assessments and care planning for in-home patients.

* Participate in the call rotation for the department, one in four nights, weekends and holidays.

* Scheduled shifts are days, Monday through Friday.
