Job description:

Job Title : Licensed Practical Nurse (LPN)
Location : New Hampshire
Shift : Nights, Evenings
Duration : 13 - 16 weeks with possible extension based on performance
Guaranteed Hours : 36 or 40/week
Pay : $40-$50/hour

Job Duties : -

Provide direct nursing care to residents in accordance with individualized care plans and facility policies.
Administer medications and treatments safely while monitoring residents for adverse reactions or changes in condition.
Monitor and document vital signs, symptoms, progress, and overall health status accurately.
Perform wound care, dressing changes, catheter care, and other skilled nursing procedures as required.
Recognize changes in residents’ conditions and promptly notify the RN, physician, or appropriate healthcare provider.
Collaborate with CNAs, RNs, physicians, therapists, and other interdisciplinary team members to coordinate resident care.
Assist with care planning, assessments, resident education, and implementation of appropriate nursing interventions.
Maintain accurate and timely documentation in electronic or paper medical records.
Promote resident safety, infection control, comfort, dignity, and compliance with LTC regulations and facility standards.
Communicate effectively with residents and families regarding care needs, health concerns, and changes in condition.
