Job Summary:

We are seeking an experienced Registered Nurse (RN) or Medical Social Worker (MSW) for a Case Management/Care Coordination assignment within a Level II Trauma Center acute care setting. The ideal candidate will support discharge planning, care coordination, utilization review, and patient advocacy across multiple hospital departments including Med Surg, ED, and other inpatient units. This role requires strong interdisciplinary collaboration, patient assessment skills, and the ability to coordinate cost-effective, patient-centered care while ensuring safe and timely discharges.

• Active RN or MSW license in Washington State (pending license accepted)
• Current BLS certification required
• Minimum 1 year of acute care experience required
• Prior discharge planning and care coordination experience required
• Experience in Med Surg, Length of Stay (LLOS), and Emergency Department strongly preferred
• Ability to float between hospital campuses and units as needed
• Knowledge of community resources, Medicare, Medicaid, DME, transportation, and rehabilitation services
• Familiarity with EPIC charting preferred
• Strong communication, documentation, and interdisciplinary collaboration skills
• Weekend rotation required (every other weekend)
• Must be comfortable working in a fast-paced acute care hospital environment
