SUMMARY:

REQUIREMENTS:

* Active RN license in practicing state required.
* Experience in SNF, LTC, or rehab preferred.
* Current BLS required; ACLS may be required.
* Strong assessment and critical-thinking skills.
* Knowledge of SNF regulations and infection control.
* Ability to provide compassionate, patient-centered care.

RESPONSIBILITIES:

* Patient Care & Assessment:

* Assess and monitor patient conditions regularly.
* Administer medications, IV therapy, and treatments.
* Develop and implement individualized care plans.

* Documentation & Compliance:

* Document all patient care and progress in the EMR.
* Ensure patient safety and compliance with policies.
* Collaborate with interdisciplinary team members.

* Education:

* Educate patients and families on care plans and health.
