Requirements:
• Bachelor's Degree in Social Work
• Active KS license
• Valid Drivers License
• One year experience with psychiatric patients preferred

Responsibilities:
• Meet with patients and the interdisciplinary team to provide updates to family and community partners.
• Gather and document information in the social service assessment and discharge plan.
• Create safe and healthy discharge plans.
• Coordinate specialized aftercare.
• Serve as point of contact for patients’ wellbeing with members not on the care team.
• Act as the key informational flow regulator regarding a patients’ progress in treatment.
• Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.
• Work effectively with fellow medical staff and treatment team members.
• Any other duties as assigned.
