Adelphi is seeking a Licensed Master Social Worker (LMSW) to provide services at a facility in Osawatomie, Kansas. The ideal candidate will deliver direct patient care services for adults receiving inpatient psychiatric treatment.

Requirements:
• Master’s Degree in Social Work
• Active KS license
• Valid Drivers License
• One year experience with psychiatric patients preferred

Responsibilities:
• Meet with patients and the interdisciplinary team to provide updates to family and community partners.
• Create safe and healthy discharge plans and coordinate specialized aftercare.
• Serve as point of contact for patients’ wellbeing with members not on the care team.
• Gather and document information in the social service assessment and discharge plan.
• Collaborate with community providers and the patient’s significant others to locate resources for discharge.
• Coordinate placement of patient to ensure that patients are released to the least restrictive environment possible, without endangering safety.
• Act as the primary liaison to families as well as the community resources, relaying information to the team in written form as well as verbally.
• Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.
• Work effectively with fellow medical staff and treatment team members.
• Any other duties as assigned.
