Registered Nurse – Home Health - Travel - (HH RN)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2420.00 - $2544.00

This Home Health RN role is based in Portsmouth, New Hampshire, a coastal community known for its charming historic downtown, picturesque waterfront, and vibrant arts and restaurant scene. The city offers easy access to beaches, walking trails, and nearby coastal towns, with a welcoming New England atmosphere and a strong sense of community. Portsmouth also provides convenient travel routes to Boston, Maine, and the broader Seacoast region, making it an attractive area for professionals who value both quality of life and career growth. In this role, you will provide skilled nursing care to adult and geriatric patients in their homes throughout Portsmouth and surrounding communities. You will conduct comprehensive assessments, develop and update individualized care plans, and coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team. A typical day may include wound care, medication management, monitoring chronic conditions, patient and family education, and safety evaluations, all delivered in the patient’s home environment. You will manage a field-based schedule that includes home visits with appropriate time allotted for documentation, communication, and care coordination. Patient acuity and caseload are managed to support safe, high-quality care and allow for meaningful one-on-one interactions. Shifts generally follow daytime hours with some flexibility depending on patient needs and agency scheduling practices. The home health setting offers the opportunity to build strong, therapeutic relationships with patients and families, support transitions from hospital or facility-based care to the home, and promote independence and quality of life. You will work as part of a collaborative, supportive clinical team that values professional judgment, clear communication, and evidence-based practice. This position is well suited for RNs who enjoy autonomy, critical thinking, and the ability to see the direct impact of their care in the patient’s own environment.
