Duties
Provide expert consultation on medication therapy management, including drug interactions, dosing adjustments, and contraindications.
Conduct thorough patient assessments to determine appropriate pharmacological interventions, considering physiology and anatomy knowledge.
Prepare and administer medications via IV infusion with strict adherence to aseptic techniques and safety protocols.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans.
Monitor patient responses to medications, including pain management strategies and critical care therapies in ICU, PICU, or trauma settings.
Educate patients on medication usage, potential side effects, and safety precautions to promote adherence and understanding.
Maintain detailed documentation of medication therapies, patient progress, and clinical interventions for accurate record-keeping.
Stay current with advances in pharmacy practice, managed care policies, and hospital protocols related to emergency room and acute care settings.
Assist in managing medication inventory, ensuring proper storage conditions for sensitive drugs such as those requiring specific handling for IV infusions.
Support hospital initiatives aimed at improving patient service quality and safety standards.
Qualifications
Bachelor's degree in Pharmacy (BPharm) or Doctor of Pharmacy (PharmD) from an accredited institution; pharmacy residency or post-residency experience preferred.
Minimum of three years of clinical pharmacy experience in hospital settings; Level I Trauma Center or Level II Trauma Center experience is highly desirable.
Demonstrated expertise in critical care environments such as ICU or emergency room settings.
Strong knowledge of pharmacology principles related to physiology, anatomy, pain management, and acute care.
