Embrace Compassionate Care in Harwood Heights Nursing Home

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our nursing home team in Norridge Park, IL. The LPN will provide direct nursing care to residents under the supervision of an RN or physician, ensuring high-quality care, resident safety, and comfort in a long-term care setting.

* Provide direct nursing care in accordance with physician orders and care plans.
* Administer medications and treatments accurately and on time.
* Monitor resident conditions and promptly report changes.
* Perform wound care, dressing changes, and basic nursing assessments.
* Record vital signs, intake/output, and other clinical data.
* Supervise and support CNAs to ensure quality resident care.
* Delegate tasks appropriately and provide guidance as needed.
* Collaborate with RNs, physicians, and interdisciplinary team members.
* Communicate effectively with residents and families.
* Complete accurate and timely nursing documentation.
* Follow facility policies, Illinois state regulations, and infection control standards.
* Assist with admissions, discharges, and care plan updates.
* Report incidents and safety concerns promptly.
* Maintain a safe, clean, and supportive environment.
* Uphold resident dignity, privacy, and rights at all times.

* Active Illinois LPN license in good standing (required).
* Long-term care or nursing home experience preferred.
* Strong medication administration and clinical skills.
* Excellent communication and leadership abilities.
* Compassionate, reliable, and team-oriented.
