RN Case Manager Hospice

The RN Case Manager holds a crucial role in directing and confirming prompt quality care for hospice patients. The RN Case Manager directs, and coordinates activities involved in hospice patient care by performing the following duties personally or through subordinate staff. Ensures regulatory oversight compliance for response, care, documentation, ongoing education/CEU, confidentiality, security, and safety. Provides sensitive and thorough education/training to patient’s primary caregiver(s) for proper participation of their care responsibilities identified in POC. Ensures the overall direction of company operations with the highest degree of company expectations, industry standards and legal compliance.

Graduation from an Accredited RN program; BSN or Advanced Education or certification preferred
 CHPN preferred.
 Maintain current Arizona RN License and any applicable CEU’s
 Maintain current CPR Certification
 Strong interpersonal and communication skills (written and oral), including the ability to lead discussions in diverse groups of varying size.
 Well organized, detail oriented and able to work independently with minimal supervision.
 Advanced level competency in electronic medical record software.
 Must possess and maintain; a valid AZ driver’s license, clean driving record, and maintain company insurance qualification of minimum coverage as follows: $100,ooo Per Person Bodily Injury, $300,000 Per Incident and $50,000 Property Damage Personal Insurance Policy.
