Key Responsibilities
Admit, discharge, and transfer patients/residents and complete associated documentation.
Obtain and report vital signs and changes in condition promptly.
Administer medications per orders and report adverse reactions.
Perform wound care and collect blood/urine specimens for testing.
Document care accurately and in a timely manner in the facility EMR.
Restock supplies, medications, and equipment to maintain unit readiness.
Communicate effectively with patients, families, and the interdisciplinary team.
Support infection control and follow facility policies and CMS guidance.
Qualifications and Requirements
Education: Graduate of an accredited LPN program.
License: Active LPN license in the state of practice (compact/multi‑state requirements should be verified).
Experience: Minimum 6 months LPN clinical experience (consider specifying 12 months for higher‑acuity settings).
Physical: Ability to push, pull, support, and lift a minimum of 80 pounds; stamina for prolonged standing/walking.
Certifications & health: CPR certification; negative TB test; annual physical where required.
