●        DEPARTMENT INFORMATION:

o   Census? Average patients/day? 75-95 per day

o   Minimum Years of Experience required? 1 year required

o   Will you accept a first-time traveler? No – must have completed at least 1 contract (13)

●        PATIENT TYPES/COMMON DIAGNOSES:

o   All types of cancers

o   3 mo. to geriatric

●        REQUIRED LICENSE, NATIONAL CERTIFICATION, CERTIFICATIONS (BLS, ACLS, NIHSS, ETC.), OR OTHER POSITION SPECIFIC REQUIREMENTS SUCH AS DRIVER'S LICENSE AND/OR INSURANCE:

o   BLS

o   ARRT-RT

●        SKILLS REQUIRED:

o   Elekta and Mosaiq experience

●        SUPPORT WITHIN THE DEPARTMENT:

o   Leadership

o   Lead Tech

o   2 Therapists per patient

●        TECHNOLOGY/EQUIPMENT:

o   EMR: Mosaiq

●        FLOATING: IF YES, WHERE?

o   Not outside of department

●        ORIENTATION (facility and unit):

o   NEO

o   Up to 1 week departmental – dependent on experience and comfort

●        SCHEDULING:

o   Weekend rotation: Mon-Fri (4-10s or 5-8s)

o   No call

o   Holiday Expectations: no major

o   RTO: Manager approval

o   Shift times: 10h 7a-5:30 or 8-5

o   Schedule cycle (how many weeks): weekly
