Physical Therapist – Home Health (Contract)
Location: De Queen, AR
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Hours: 40 hrs/week

Job Overview:
We are seeking a licensed Physical Therapist to provide home health services in the De Queen, Arkansas area. This role focuses on evaluating patients, developing individualized treatment plans, and delivering rehabilitation services to improve function and independence in the home.

Coverage Area:
Howard, Sevier, Hempstead, Pike, Little River, Miller, and Pope counties

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop, implement, and modify individualized treatment plans
Provide therapeutic exercises and functional rehabilitation in patient homes
Communicate patient progress with physicians, care teams, and families
Complete accurate and timely clinical documentation
Supervise PTAs and aides in accordance with state practice acts

Active Arkansas Physical Therapist license (compact accepted)
Home health experience preferred
OASIS and PointCare experience a plus
