Physical Therapist

Job Title: Physical Therapist (PT)
Location: Plymouth, NC 27962
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday 8:00 AM – 5:00 PM (No weekends or on-call required)
Mileage Reimbursement: $0.43 per mile
Perform physical therapy evaluations and treatments per physician orders and referrals
Conduct comprehensive assessments using appropriate evaluation tools and clinical judgment
Review patient diagnoses, medical history, precautions, and contraindications
Develop individualized, measurable treatment plans to optimize patient independence and functional outcomes
Reassess patient progress regularly and modify goals and treatment plans as needed
Communicate patient needs and progress with physicians, interdisciplinary teams, patients, and family members

Experience with Homecare Homebase (HCHB) strongly preferred
Home health physical therapy experience preferred
License/Certification:
Physical Therapy License (Required)
