● ABOUT FACILITY:
▪ Home Health
▪ RADIUS RULE: Traveler’s permanent address must be 50 miles outside of home unit
● DEPARTMENT INFORMATION:
o 6 visits a day
o There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
o Minimum Years of Experience required? 1 year Home Health
o Will you accept a first-time traveler? Yes, if seasoned to home health
● PATIENT TYPES/COMMON DIAGNOSES:
o All Adults
o Chronic diseases, Orthopedics/Ortho Surgery
o Teammates will travel throughout the below areas:
▪ Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between
● REQUIRED LICENSE, NATIONAL CERTIFICATION, CERTS (BLS, ACLS, ETC.), OR OTHER POSITION SPECIFIC REQUIREMENTS SUCH AS DRIVER'S LICENSE AND/OR INSURANCE:
o DRIVER'S LICENSE AND PROOF OF INSURANCE REQUIRED
o PT: BLS; State Licensure required
o OT: BLS; State Licensure required

o 5 days orientation
● SCHEDULING:
o Weekend rotation none
o No call
o No major holidays
o RTO: Manager approval
o Schedule cycle: every 13 weeks

* Do they schedule their own appointments with this position? - They will schedule the visit with the patients the day prior to the visit planned.
* Do they still get paid for cancelled appointments? Cancelled appts are not counted as visit equivalents. They are required 30 VE a week

* Is a company cell phone provided? A phone is provided and is used for documentation and calling patients/providers.
