Job Summary

We are seeking a compassionate and dependable Licensed Nursing Assistant (LNA) to join our healthcare team on a per diem basis. The LNA will provide direct patient care and assist nursing staff in delivering high-quality care while promoting patient comfort, dignity, and safety.

Key Responsibilities
Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
Take and document vital signs, height, weight, and intake/output.
Assist patients with mobility, transfers, ambulation, and range-of-motion exercises.
Observe and report changes in patient condition to the supervising nurse.
Maintain accurate and timely documentation in the electronic medical record (EMR).
Ensure patient rooms are clean, safe, and organized.
Respond promptly to patient call lights and requests.
Assist with admissions, transfers, and discharges.
Follow infection prevention, HIPAA, and facility safety policies.
Collaborate with nurses and the interdisciplinary healthcare team to provide quality patient care.
Qualifications
Active New Hampshire Licensed Nursing Assistant (LNA) License.
Current BLS (Basic Life Support) Certification preferred.
Minimum 6 months to 1 year of recent LNA experience preferred.
Experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, or hospital settings is preferred.
Strong communication, teamwork, and patient care skills.
Ability to work independently and in a fast-paced environment.
