Physical Therapist – Home Health (Contract)
Location: Covington, GA
Schedule: Monday–Friday
Hours: 40 hrs/week (32 hours guaranteed)

Job Overview:
We are seeking a licensed Physical Therapist to provide home health services in the Covington, Georgia area. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver rehabilitation care to help patients improve mobility, function, and independence in the home setting.

Coverage Area:
Newton and Henry counties

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop and adjust individualized treatment plans
Provide therapeutic exercises and functional rehabilitation in patient homes
Communicate patient progress with physicians, care teams, and families
Complete timely and accurate clinical documentation
Supervise PTAs and aides as required

Active Georgia Physical Therapist license
Home health experience preferred
HCHB experience a plus
