Physical Therapist – Home Health (Contract)
Location: Dublin, GA
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Hours: 40 hrs/week

Job Overview:
We are seeking a licensed Physical Therapist to provide home health services in the Dublin, Georgia area. This role focuses on evaluating patients, creating individualized treatment plans, and delivering rehabilitation care to improve mobility, function, and independence in the home setting.

Coverage Area:
Laurens County

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop and adjust individualized treatment plans
Provide therapeutic exercises and functional rehabilitation in patient homes
Communicate patient progress with physicians, care teams, and families
Complete timely and accurate clinical documentation
Supervise PTAs and aides as required

Active Georgia PT license (compact accepted)
Home health experience preferred
HCHB and OASIS experience a plus
