Job Title: Registered Nurse (RN) Clinical Caregiver Home Health – Day Shift

Location: Denver, CO 80210 (RN will cover about 30-40 mile radius)

3-Month Contract
Full-Time
Schedule: Wednesday-Saturday 7am-5:30pm
RN Key Responsibilities:

Triaged participant and staff calls, prioritized care needs, and routed appropriately
Administered medications and treatments; monitored compliance and responses
Provided skilled nursing care in participants’ homes, including preventive and rehabilitative interventions
Assessed and reassessed participants to identify changes in condition and adjust level of care as needed
Educated participants and families on medications, treatments, and health management
Documented care accurately in medical records and completed required charting and progress notes
Coordinated interdisciplinary care and made referrals to PT, OT, SLP, and Social Work
Conducted care conferences and collaborated on care planning with home care leadership
Participated in quality management, utilization review, and peer review activities

REQUIRED

Current State issued Registered Nurses License
Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
1+ RN experience year working with the geriatric population
