JOB DESCRIPTION & REQUIREMENTS
Registered Nurse – Case Management- Case Manager - Travel - (CM RN)
StartDate: 9/22/2026 Available Shifts: 8 D Pay Rate: $3262.00 - $3429.00

Required Qualifications
Registered Nurse, Case Manager SNF - geriatric (discharge planning, resources)
SNF guidelines/experience preferred
Customer service - interpersonal skills
Case management experience required
BSN preferred
*SOME TRAVEL REQUIRED*
Home base Manteca, will travel to Stockton mainly, sometimes to Modesto/CVA
1-2 days at assigned facility, other 3 days in office
