JOB REQUIREMENTS

* 2+ years of experience

* Current and unrestricted APN and RXN license in Colorado required.

* National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other appropriate population focus preferred

* DEA registration

* BLS/CPR certification

* Valid driver's license, acceptable driving record, and proof of automobile maintained throughout employment.

JOB RESPONSIBILITY

* Provides a wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, and urgent medical needs all in collaboration with the Chief Medical Director.

* Conducts health assessments and evaluates patient needs in the clinic

* Actively collaborates with the entire Medical interdisciplinary team for the highest quality care of each participant.

* Make appropriate and timely referrals to our contracted providers out in the community
