Position Overview
The Registered Nurse (RN) plays a critical role in delivering skilled nursing services to patients in their homes. This role involves comprehensive patient assessment, care planning, OASIS documentation, coordination of services, and supervision of field staff as applicable.
This is an ideal role for a nurse who values autonomy, clinical excellence, and meaningful patient relationships.
Key Responsibilities

Conduct comprehensive skilled nursing visits including admissions, recertifications, resumptions of care, and discharges
Complete accurate and timely OASIS documentation
Develop, implement, and update individualized Plans of Care in collaboration with physicians
Assess patient conditions, monitor progress, and identify changes requiring intervention
Educate patients and caregivers on diagnoses, medications, treatments, and home care techniques
Coordinate care with therapists, case managers, physicians, and support staff
Supervise and provide clinical oversight to LPNs and HHAs as assigned
Ensure compliance with Medicare, Medicaid, and agency policies
Participate in case conferences, quality improvement initiatives, and required trainings

Qualifications

Active Missouri Registered Nurse (RN) license
Minimum of 1 year RN experience (home health experience preferred)
Strong clinical assessment and documentation skills
Familiarity with OASIS and home health regulations
Excellent communication, organization, and time‑management skills
Ability to work independently while collaborating with a team

The compensation range outlined above is a fair and market‑aligned estimate based on anticipated visit volume, admissions, and per‑visit compensation structure. Final earnings may vary based on workload, census, and performance.

Qualifications

Active and unrestricted Registered Nurse (RN) license in the State of Missouri
Minimum of 1 year of professional nursing experience (home health experience strongly preferred)
Demonstrated knowledge of Medicare and Medicaid home health regulations
Experience completing or supporting OASIS assessments (admissions, recertifications, discharges)
Strong clinical assessment, critical‑thinking, and patient education skills
Ability to work independently in the field while collaborating effectively with an interdisciplinary team
Excellent documentation, communication, and time‑management skills
Comfortable supervising and providing clinical guidance to LPNs and HHAs as assigned
Reliable transportation and ability to travel to patient homes within the service area
