Make a Difference: RN Role in Caring Community!

Sunflower Adult Day Health Center in Bayonne is seeking a compassionate, dependable nurse to join our small, close-knit care team. In this daytime role, you'll provide health monitoring, medication management, and personalized care for older adults and individuals with chronic conditions building real relationships with the members we serve.

This is a Monday-through-Friday position with no overnight shifts, in a warm, non-corporate environment where your clinical judgment is valued and you're known by name.

What you'll do:
Monitor participant health and document changes
Manage medications and coordinate care plans
Collaborate with aides, therapists, and activity staff
Support participants' wellbeing and daily engagement


What we're looking for:
Current RN in NJ
A caring, patient-centered approach
Strong communication and teamwork skills
Experience with older adults and/or chronic care a plus
