Flexible Home Health RN Shifts Available in Groves!

Position: PRN/ Per Diem
Schedule: Flexible
Service Area: Groves and Surrounding areas ( including Beaumont and Orange)

365Care Home Health is seeking a dependable and compassionate Registered (RN) to join our team on a PRN/ Per Diem basis in Groves, TX and surrounding areas.

The Home Health RN will provide skilled nursing care to patients in the comfort of their homes while working closely with physicians, caregivers and other members of the interdisciplinary team. This position offers a flexible schedule and is ideal for an experienced nurse who is comfortable working independently in the home health setting.

Responsibilities include:
Perform skilled nursing visits and comprehensive patient assessments.
Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits as assigned
Provide wound care, wound VAC management, IV therapy, blood draws, medication management, and other skilled nursing service.
Monitor patients for changes in condition and communicate concerns promptly to physicians and the clinical team.
Provide patient and caregiver education
coordinate care with physicians, therapists, and other health care professionals
complete all clinical documentation accurately and within required timeframes.
Follow the patient's Plan of Care and all applicable home policies and regulations

Current, unrestricted Texas Rn license in good standing (required)
Minimum of 2 years of nursing experience strongly required
Home health experience (preferred)
Experience with wound care, Wound VACs, IV therapy, and blood draws (required).
OASIS experience (Preferred)
Other Requirements:
Strong clinical assessment and documentation skills
Reliable transportation and valid driver's license
Ability to travel to patient's homes within the assigned services areas
Must be dependable and able to complete documentation in a timely manner
