REQUIREMENTS:

* Active LVN/LPN license in the practicing state
* Graduate of an accredited LVN/LPN program
* BLS certification
* Skilled Nursing or Long-Term Care experience (preferred)
* Knowledge of state nursing regulations and SNF protocols
* Ability to lift, reposition, and assist residents as needed

RESPONSIBILITIES:

* Provide direct patient care according to care plans and physician orders
* Administer medications and treatments as prescribed
* Monitor and document patient conditions, vital signs, and changes in status
* Assist with admissions, discharges, and transfers
* Perform wound care and assist with rehabilitative services
* Collaborate with RNs, physicians, and interdisciplinary care teams
* Educate residents and families on care plans and treatments
* Maintain accurate and timely documentation
* Follow facility policies, state regulations, and infection control standards
