Job Description:
Perform RN Case Manager duties in a hospital setting
RN Case Manager case load will vary from facility to facility
Assist with hospital discharge planning
Some roles include utilization review/management duties
Complete initial assessments and plan of care
Pay package will be all-inclusive and will vary from state to state
