StartDate: ASAP Available Shifts: 12 D Pay Rate: $2762.00 - $2932.00

Required Qualifications
2 years of Case Management, Acute Care Experience, direct patient care delivery.
Exp in utilization review/Case mgt, and discharge planning, care coordination across the continuum of care in collaboration with the physician and other members of the health care team.
Discharge Planning, explanation of benefits, Kaiser Health Connect, Care Port.

Preferred Qualifications
ICU/ED Experience
Health Connect/Epic experience
