* Work History going back at least 3 years, if applicable to Assignment
* Skills Checklist
* At least '1' supervisor reference included.... 2nd to be obtained during onboarding
* Licenses & Certifications
* COVID Vaccination card w/ booster, if applicable
* California license

* BLS (AHA)
