The Registered Nurse position will be an integral component of the agency’s Administrative Team and will work closely with Program Managers to oversee the general health and physical welfare of the individuals served. The Registered Nurse will abide by licensed nursing standards, agency guidelines, and follow State protocols and DDS guidelines. Oversee medical needs for about 8 different locations all located in southern CT on the shoreline.

Duties include:
Assist the Program Managers with coordination of medical care for individuals with physical and intellectual disabilities.
• Perform annual pre-physical nursing assessments for individuals served.
• Attend necessary medical appointments and/or provide guidance to staff for information needed at the medical appointments.
-Attend DDS Nursing meetings as required
• Complete Nursing Health and Safety Assessment upon intake or transfer of residents.
• Complete Nursing assessment within 24 hrs. of an individual returning home from hospital or skilled nursing facility.
• Participate in the individuals' Individual Planning meetings and be an active member of the planning and support team for individuals. Complete quarterly nursing reports per DDS.
• Complete annual nursing assessments including self-medication assessment, Fall Risk Assessment, Dysphagia guidelines, and any other assessments as required by DDS.
• Administer annual flu shots as needed.
• Review Health history and Diagnosis lists annually to ensure accuracy.
• Ensure at minimum there is a monthly nursing note for all individuals on caseload.
• Review MARs at least monthly to ensure there are no errors.
• Perform and document quarterly medication audits.
• Complete medication error retraining with staff in a timely manner.
• Complete staff training to include annual nursing delegations, annual pass and pours, medication review, dysphagia training, and any other required training per DDS.
• Review individuals’ medical records regularly to ensure DDS compliance with minimum routine preventive care guidelines and that follow up is being completed timely per doctors’ orders.
• Ensure all doctor orders are current, signed by provider, and accurate with no lapses in timeframe.
• Ensure lab work is completed as ordered by the providers.
• Ensure psychotropic medication orders are being reviewed as scheduled, AIMS, and Prescriber Treatment Plans are current
-Serve as member of agency committees as needed.
-Other tasks as assigned by supervisor of nursing, Executive Director, or Assistant Executive Director.

-Valid state of CT Registered Nurse license required
-On-call during working hours for agency
-Valid CT drivers license with clean driving record required. Must have a vehicle to get to different sites (Mileage reimbursement offered).
-Strong written and verbal communication skills
-Professional and team-oriented a necessity.
-Ability to navigate a computer with some proficiency.
-Experience with Therap, Microsoft office, Teams, etc. a plus
