Job Summary:
The Physical Therapist is responsible for evaluating, diagnosing, and treating patients with physical impairments, functional limitations, or movement dysfunctions. The PT develops and implements individualized treatment plans to help patients improve mobility, reduce pain, restore function, and prevent disability.

Key Responsibilities:
• Evaluate patients’ physical conditions, mobility, strength, balance, and range of motion
• Develop and implement individualized treatment plans based on physician referrals
• Provide therapeutic exercises, manual therapy, and modalities as appropriate
• Educate patients and caregivers on treatment plans, exercises, and injury prevention
• Monitor patient progress and modify treatment plans as needed
• Maintain accurate and timely clinical documentation in EMR systems
• Collaborate with physicians, nurses, and other healthcare professionals
• Ensure compliance with federal, state, and facility regulations
• Maintain a safe and clean treatment environment

Qualifications & Requirements:
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Active Physical Therapist license in the practicing state
• CPR/BLS certification (preferred or required by facility)
• Strong clinical assessment and patient care skills
• Ability to work in post-acute / skilled nursing or rehab settings
