REQUIREMENTS:

* Graduate of an accredited Nurse Practitioner program with a Master’s degree in Nursing – [REQUIRED]

* Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED]

* Unrestricted license to practice as a Physician Assistant in the State of Colorado – [REQUIRED]

* Must be eligible for DEA and Medicare/Medicaid provider status — [REQUIRED]

* Clean malpractice/license history - [HIGHLY PREFERRED]

* Must have no sanctions/exclusions from HHS/OIG or GSA — [REQUIRED]
