JOB SUMMARY
The Registered Nurse Case Manager (RNCM) is responsible for managing the care of pediatric clients, according to their individual needs and care plan using a multidisciplinary method. The RNCM facilities referrals to primary providers, specialists, healthcare facilities, and community resources, as needed. The RNCM is responsible for direct client care including a holistic approach to the care of the children and their families, including educational, psychosocial, behavioral, physical, mental, and emotional care/needs of the child. The RNCM will educate the client and family regarding immediate care needs and preventative behaviors.

Demonstrates the ability to provide individualized care at all times.
Utilizes the appropriate guidelines and information based on the age of the child to foster independence and advancement.
Handles emergent situations safely while remaining calm.

Demonstrates good assessment, evaluation and observation skills.
Performs multiple tasks prioritizing these tasks appropriately.
Flexible in changes that occur throughout the day including staffing, children, employees, and nursing care. Reports situations immediately to the SDON.
Maintains the following nursing responsibilities:
Coordinates all care of children as directed by the physician according to the Plan of Treatment and Care Plan.
Makes all judgments based on a holistic approach to the care of the child.
Provides direct client care including but not limited to personal care activities, use of adaptive equipment, follow-through for therapy, and administration of medications, treatments, and procedures.
Promotes an environment which supports and encourages client/family advocacy.
Maintains a clean, healthful, and safe environment.
Documents appropriate information in a timely manner.
Performs chart documentation at the time care/direction is given.
Utilizes proper procedure for all care/direction.
Documents or delegates this documentation for accurate and complete demographic assessment information on all children.
Attends in-service education to stay up to date with evidence-based practice and to learn new procedures, applications, and updates.
WORK CONDITIONS:
While performing the duties of this job the employee is required to execute activities on a child’s level. This includes sitting and crawling.
The employee may be confined to a limited workspace. The employee is required to bend, stoop, squat, kneel and reach freely.
The employee must be able to lift or move a maximum of 50 – 100 lbs with assistance.
The employee is required to assist with transfers, total lifts, walking, position changes and sitting.
Specific vision abilities required by this job include close vision, color peripheral vision and the ability to adjust focus.
The employee must have acute hearing and olfactory senses. Employees must have good tactile senses and fine manual dexterity with range of motion to all extremities.

QUALIFICATIONS
Graduate of an accredited school of nursing with current RN licensure.
Must be licensed in the state of the designated facility or, if applicable, have a qualifying Nurse Licensure Compact (NLC).
Basic personal computer skills and comfort with Microsoft Office.
Preferred: At least six months of current experience as a practicing RN with pediatric experience.
Maintains a current valid RN license in the state of the designated facility.
Current CPR (Basic Life Support) is required.
Current TB test required.
