LPN Home Health Care (working in the home)

We are seeking LPN's for a one-on-one care environment focused on personalized support and compassionate patient care. This role includes medication administration, care planning, patient monitoring, companionship, errands, transportation, and assistance with daily household needs. All shifts are available Monday through Sunday, typically between 8 a.m. and 8 p.m., with flexible scheduling (we're looking to hire multiple nurses for this job). Occasional local driving is required for errands and transportation (must have a valid driver's license).

Responsibilities

Administer medications and maintain medication administration records.
Develop and follow individualized care plans.
Monitor patients and document changes in condition.
Provide compassionate one-on-one care and companionship.
Assist with activities of daily living.
Provide occasional local transportation and errand assistance.
Support light housekeeping, laundry, and meal prep.
Coordinate shifts and document care using electronic health records and a mobile caregiver platform.


Required Qualifications

Entry-level experience in practical nursing.
Practical Nursing Diploma or Certificate.
Current Licensed Practical Nurse (LPN) license.
Knowledge of medication administration, patient monitoring, and care planning.
Experience supporting activities of daily living and patient transportation.
Ability to complete documentation accurately.
Compassionate care and effective communication skills.
