PATIENT INFORMATION

* PATIENT DEMOGRAPHICS: Adult Oncology/Hematology patients

* PATIENT VOLUME: 18–20 patients per day

* PRACTICE SETTING: Outpatient Oncology Clinic with occasional inpatient consults

* INFUSION CHAIRS: 15

* TREATMENTS & PROCEDURES:

* Chemotherapy

* Immunotherapy

* Targeted Therapy

* Hormone Therapy

FACILITY INFORMATION

* CURRENT PROVIDER TEAM:

* 1 Physician

* 1 Nurse Practitioner

* 3 Clinical Support Staff (CNA/CMA)

* EMR: Epic (AI-assisted charting available)

PRIVILEGES & COMPLIANCE

* BOARD CERTIFICATION: Board Certified or Board Eligible accepted

* LICENSE: Active New York Medical License required

* EMR EXPERIENCE: Epic experience required

REQUIRED PROCEDURES

* Chemotherapy administration and management

* Immunotherapy management

* Targeted therapy management

* Hormone therapy management

* Oncology patient evaluation and follow-up

* Occasional inpatient oncology consults

JOB REQUIREMENTS

* Active New York Medical License

* Board Certified or Board Eligible in Hematology/Oncology

* Epic EMR experience required

DUTIES & RESPONSIBILITIES

* Provide comprehensive outpatient Hematology/Oncology care.

* Evaluate, diagnose, and develop treatment plans for oncology patients.

* Administer and manage chemotherapy, immunotherapy, targeted therapy, and hormone therapy.

* Document patient encounters using Epic EMR with AI-assisted charting.

* Collaborate with physicians, advanced practice providers, and clinical staff to deliver high-quality patient care.

* Perform occasional inpatient oncology consultations as needed.

* Maintain timely documentation and comply with facility policies and clinical standards
