REQUIREMENTS:

* 2+ years of experience
* Current RN license
* BLS certification

DUTIES & RESPONSIBILITIES:

* Provide compassionate care, strength and courage to patients and families.
* Help patients identify personal goals of care and help caregivers gain confidence.
* Serve as generalists on all diseases and specialists in symptom management.
* Lead the interdisciplinary team for assigned caseload of patients.
* Provide care in various settings: private homes, nursing homes, group homes, or assisted living facilities.
* Work weekday schedule Mon-Fri 8am-5pm with occasional back-up for weekends and on-call.
