Physical Therapist

Perform comprehensive physical therapy evaluations and treatments per physician orders
Review patient diagnoses, medical history, precautions, and contraindications
Develop individualized, measurable treatment plans to maximize patient independence and function
Reassess patient progress regularly and modify treatment plans as needed
Communicate patient status and progress with physicians, care teams, patients, and families
Coordinate discharge planning and consult with medical providers as appropriate
Recommend and assist with adaptive equipment to enhance patient function

Exercise programming and rehabilitation
Home health experience required
Proficiency with HCHB / OASIS documentation
Strong clinical assessment and care-planning skills
License/Certification:
Physical Therapy License (Preferred)
