REQUIREMENTS:

* Current Registered Nurse (RN) license in the state of Utah or compact state RN license.
* Minimum of 3 year of nursing experience, preferably in hospice, home health or palliative care.
* Strong clinical assessment skills.
* Excellent communication, interpersonal, and organizational skills.
* Compassionate demeanor and ability to support patients and families during difficult times.
* Valid driver’s license and reliable transportation required for home visits. Mileage reimbursement $.70 per mile.
* Current BLS Certification
* On-Call 1-3 shifts per week, to include 3 weekends during the 13 week contract depending on staffing levels. Able to respond within 45 minutes to patient needs when on call.
* Able to work holiday
* Hospicesoft/careficient EMR experience preferred
* Requires successful completion of a pre-employment drug test and background check.
* 3+ years of experience

DUTIES & RESPONSIBILITIES:

* Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
* Provide pain and symptom management consistent with the hospice plan of care.
* Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
* Collaborate with the interdisciplinary team including physicians, social workers, chaplains and volunteers.
* Participate in on-call rotation
* Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.
