HOME HEALTH RN - PER DIEM

Gabone Home Health is seeking compassionate, reliable, and experienced Registered Nurses to join our growing home health team throughout Los Angeles County.

Responsibilities:

Conduct skilled nursing visits in patients’ homes.
Perform Start of Care (SOC), Resumption of Care, Recertification, Follow-Up, and Discharge visits.
Complete comprehensive nursing assessments.
Administer medications and provide wound care as ordered.
Coordinate patient care with physicians and interdisciplinary team members.
Educate patients and caregivers regarding treatment plans.
Complete accurate and timely clinical documentation.
Compensation:

Follow-Up Visits: $90 per visit
SOC / Recertification / Discharge: $130 per visit
Schedule:

Per Diem / Flexible Scheduling
Daytime Patient Visits

Active California Registered Nurse (RN) license in good standing.
Minimum 1 year of nursing experience preferred.
Home health experience strongly preferred.
Familiarity with OASIS documentation preferred.
Strong clinical assessment and communication skills.
Reliable transportation and valid driver’s license.
Ability to work independently and manage patient visits efficiently.
Strong organizational and documentation skills.
Familiar with the EMR Synergy
