In this dynamic role, you will integrate care management, social services, discharge planning, and utilization review to ensure the best possible outcomes for patients—both during hospitalization and beyond. You’ll collaborate with an interdisciplinary team to reduce care fragmentation, improve patient experiences, and support smooth transitions from hospital to home or other care settings.
As an RN Case Manager, you will:
-Coordinate and facilitate comprehensive care plans
-Support discharge planning and post-hospital transitions
-Provide patient and family education
-Manage utilization review to ensure appropriate, efficient care
-Advocate for patients and connect them with needed resources
-Collaborate with providers and community partners

Education
- Graduate of an accredited school of nursing.
Licensure
- Wyoming RN licensure (eligible to be licensed)
