Physical Therapist – Home Health (Contract to Hire)
Location: Owensboro, KY
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Hours: 40 hrs/week

Job Overview:
We are seeking a licensed Physical Therapist to provide home health services in the Owensboro, Kentucky area. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver rehabilitation care to improve mobility, function, and independence in the home setting.

Coverage Area:
Daviess, Ohio, McLean, Henderson, and Webster counties

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop and adjust individualized treatment plans
Provide therapeutic exercises and functional rehabilitation in patient homes
Communicate patient progress with physicians, care teams, and families
Complete timely and accurate clinical documentation
Supervise PTAs and aides as required

Active Kentucky PT license (compact accepted)
Home health experience preferred
Homecare/Home Health and OASIS experience a plus
