This Home Health RN opportunity is based in Burlington, North Carolina, a vibrant community ideally situated between the Triad and Triangle regions. As a Home Health RN in this role, you will provide skilled nursing care to patients in their homes throughout Burlington and surrounding communities. You will perform comprehensive assessments, manage chronic and acute conditions, administer medications and treatments, monitor for changes in patient status, and collaborate with an interdisciplinary team to coordinate care. Patient populations often include adults and seniors recovering from surgery, managing complex chronic diseases, or seeking to maintain independence safely at home. You can expect to manage a daily caseload of home visits within a defined territory, usually focusing on a reasonable number of patients that allows for thorough assessments and quality documentation. Your typical day may include morning and afternoon visits to patients in different neighborhoods, completing assessments, providing education, and coordinating with physicians and other clinicians. Documentation is completed using an electronic medical record, often via a laptop or tablet, allowing you to complete charting efficiently between visits or at the end of the day. Responsibilities typically include: Conducting initial and follow-up nursing assessments in the home. Developing and updating individualized care plans in collaboration with the patient, family, and care team. Providing direct clinical care such as wound care, medication administration, IV therapy where appropriate, and monitoring vital signs. Educating patients and caregivers on medications, disease processes, symptom management, and safety in the home environment. Coordinating with physical, occupational, and speech therapists, social workers, and home health aides to ensure cohesive, patient-centered care. Communicating with physicians and other providers regarding changes in patient condition and recommending adjustments to the plan of care when necessary. Ensuring timely, accurate documentation and compliance with regulatory and agency standards. Schedules in home health are typically day-shift based, with flexibility needed to accommodate patient needs and occasional weekend or holiday rotations.
