DUTIES & RESPONSIBILITIES:

* Provide compassionate care, strength and courage to patients and families.
* Serve as a teacher, helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member.
* Act as a generalist on all diseases and a specialist in symptom management, requiring excellence in assessment and critical thinking.
* Lead the interdisciplinary team caring for an assigned caseload of patients.
* Provide care wherever patients call home - private homes, nursing homes, group homes or assisted living facilities.
* Work a weekday schedule of Mon-Fri 8am-5pm, with occasional back-up for weekends and on-call.
